Before your first step, the recipe runs: geography, factions, wars, artifacts, named beasts, plagues, and the occasional thing that falls screaming from the sky. Cities burn and become the dungeons you'll loot. Then you're born into the wreckage with six stats, a rusty sword, and no expectations.
